I hereby certify that myself/my child is in good physical condition and is able to participate fully in this program. All current medical conditions requiring medication are indicated on form. I release Artists’ Play and its teachers from liability in the case of accident or injury. I understand that all instructors are professionally trained and will conduct all classes in the safest possible manner.
CHILDREN’S CIRCUS AND DANCE CLASSES/CAMPS: There are no refunds for missed classes.
Refunds will be determined on time of cancellation; If you require a refund one month before the program starts you will be refunded the amount less a $25.00 admin fee. Two weeks before; less $25.00 admin plus two weeks of class fee. Less than two weeks notice; $25.00 admin fee plus one month of class fees. After the class has started there will be no refund. We are happy to transfer your child into another class.
Payment Policies
Payment is due upon registration. For families with multiple classes and multiple children in classes there is a 5% discount applied when you register. Installments plans may be considered for multiple classes. Please contact the office to make special arrangements.
ADULT DROP-IN CLASS CANCELLATION POLICY:
Your class must be cancelled twenty-four hours (or more) in advance of the class start time to receive a refund.
Classes cancelled less than 24 hours in advanced will NOT be refunded.

I hereby agree to ensure Artists’ Play emails are not ending up in my junk mail. I promise to log in to my parent portal and make payments as needed, ensure my child is enrolled in all terms I require.
If I require assistance I will email andrew@artistsplay.com and ensure I have access to my account.
All of our classes require proper attire to be worn to ensure proper movement and safety of the students and the teachers.
HAIR:
Long hair must be pulled back into a ponytail for ALL classes at Artists’ Play. Please ensure you have a hair tie. We now sell them for .50 cents each. Students will not be able to participate if their hair is not tied back. Accounts will be charged for hair ties.
CLOTHING:
Aerial:
legs, mid-section and armpits shall be covered. Please ensure if they are wearing crop tops that the leggings meet the height of the crop top. Students can bring an extra sleeved shirt to avoid armpit burns. Please ensure long leggings are worn for all aerial classes. All clothing should be fitted and not able to move when going upside-down.
NO ZIPPERS!!! NO JEWELLERY!!! Zippers will put holes in our silks, jewellery will get caught and cause injuries or holes in the silks.
Acro:
All clothing should be fitted and not able to move when going upside-down.
shorts can be worn for this class but they must be fitted and have full coverage.
Ballet:
By level 3 we would like all of our dancers to have a bun in their hair for ballet.
All ballet classes require ballet bodysuit, ballet tights, ballet shoes and an optional ballet skirt.
Jazz/Lyrical:
fitted clothing and jazz shoes
Modern:
Fitted clothing and bare feet
Hip Hop:
Shirt, shorts or pants able to move in and INDOOR runners

We come each day prepared to create a safe environment. Please ensure you or your child/children are prepared to come to the studio to do their best. To communicate when they need extra help or when they feel they need more challenge.
Coming to class on time:
You and your child(ren) agree to arrive for class ready to participate at the time of class and to follow the Dress Code policy. Arriving late means they are not warming up properly for class and is a safety issue. We understand things happen and lateness will happen sometimes. If a student is more than 15 minutes late for a class they may not be able to participate fully.
I agree that continue to be bound by all previous agreements and waivers I have signed with Artists’ Play and will conduct myself in a manner befitting of those agreements.

Important information about aerial silks at home
Below is all the information about Camps, drop-off times, pick-up, what to wear, and who to call. If you have more questions please don’t hesitate to email or call or text my cell phone.
Location of the studio:
We are located inside 388 Carlaw Ave, unit 205.
ENTER FROM FRONT OF BUILDING – 388 CARLAW: If you enter from the main doors (glass door with metal siding above) with the elevator and go to the second floor you will find a line of pink tape to help guide you. If you enter through a green door under our signage follow the signs to go up the stairs and then follow the pink tape.
Pick up and Drop off will always be at our Studio doors inside the building.

Alternative ways to enter the building: Back of building
Just off of Dickens (the street just north of Dundas that looks like a laneway), you will find the back of the building, there is Rock Oasis and Primal Fitness as well as signs for Desmond and Beatrice. You will see a sign for Artists’ Play as well on the wall beside the garage door you will enter and follow our signs! Please do not try to enter through Rock Oasis. There is no parking available in this area. Please do not park in this area as it is used for deliveries and we don’t want to impede other businesses. There is paid parking on Dickens, and free parking on Logan and other side streets in the area.
Drop off and pick up times:
Drop off will be staggered between 8:40 and 9am. Pick up will be staggered between 4pm and 4:15pm
Students doing both morning and afternoon will stay for lunch.
Drop off and pick up will be at our main door inside the building.
Group 1 is Half Day (can be in both half days) ages 4 to 6, Group 2 is ages 7 to 11, Group 3 is ages 9 and up.
